Deposits

I UNDERSTAND THAT DEPOSITS ARE NON-REFUNDABLE

Deposits are applied towards the cost of your tattoo on your final session

Deposits are transferable between artists/services in the studio with a minimum of (2) two weeks’ notice prior to your first appointment


Rescheduling

A minimum of 72 HRS notice is required to reschedule your appointment

Rescheduling must be done over the phone during business hours and a new date must be scheduled at that time

Any emails or phone calls received after hours will not be considered valid.

The Rescheduling Policy applies to a maximum of rescheduling (2) two times before your deposit is forfeited regardless of notice given.

In the event of a last-minute cancellation, scheduling conflict or change of heart, your deposit will be forfeited.


Design

RIGHT OF REFUSAL

Our artists have the right to refuse any tattoo if the design is changed outside of what purchased online.

Any major changes to your overall design must be approved by your artist at a minimum of (2) two weeks in advance of your first appointment.


Exchanges

Deposits are transferable between artists/services in the studio with a minimum of (2) two weeks’ notice prior to your first appointment and must be initiated within 30 days of booking.


Additional Disclosures

I CONFIRM THAT I AM OVER THE AGE OF CONSENT FOR THIS PROCEDURE. IF UNDER THE AGE OF CONSENT, I CONFIRM THAT MY LEGAL GUARDIAN WILL BE PRESENT ON THE DATE OF THE PROCEDURE AND PHOTO ID OF ALL PARTIES IS PRESENTED & PHOTOCOPIED. ALL IDENTIFICATION(S) OF GUARDIAN(S) MUST HAVE THE SAME ADDRESS. IF MY GUARDIAN IS NOT PRESENT, THE PROCEDURE WILL NOT BE CARRIED OUT AND MY DEPOSIT WILL BE CONSIDERED VOIDED.

I HEREBY ACKNOWLEDGE THAT BY LEAVING A DEPOSIT, I UNDERSTAND THAT IT IS THE OBLIGATION OF THE ABOVE PRACTITIONER TO COMPLETE MY PROCEDURE AND THAT I KNOWINGLY ASSUME ALL LEGAL AND FINANCIAL RISK IN THE CASE THAT MY PRACTITIONER IS UNABLE TO COMPLETE MY PROCEDURE AT THE SCHEDULED DATE AND/OR TIME OF MY APPOINTMENT(S) WHATEVER THE REASON MAY BE. SHOULD THIS SITUATION OCCUR, I UNDERSTAND THAT MY DEPOSIT IS NON-REFUNDABLE AND TIMELESS INK WILL MAKE ALTERNATE ARRANGEMENTS TO HAVE MY PROCEDURE COMPLETED IN STUDIO BY ANOTHER PRACTITIONER. SHOULD I CHOOSE TO DIRECTLY CONTACT ABOVE PRACTITIONER TO COMPLETE MY PROCEDURE AT ANOTHER LOCATION, IT IS AT MY OWN RISK.


BY LEAVING A DEPOSIT, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD THE DEPOSIT AND CANCELLATION POLICY AND THAT ALL OF MY QUESTIONS REGARDING THE DEPOSIT AND BOOKING POLICY HAVE BEEN THOROUGHLY ANSWERED BY TIMELESS INK.
